You must fill out and return all three of the following forms by 4/15/13 to:

1. Availability Form (Spring 2013) - See Link Below for Form

2. Medical Waiver Form (if under 18 yrs) – Valid for 1yr. See Link Below for Form.

3. Referee Information Form –Valid for 1 yr.  See Link Below for Form

Returning referees:

You must fill out and return the Fall availability form and return to Mrs. Drizlane by 4/15/13. You can email her @ EDriz47@comcast.net

If you will be involved in other activities (school clubs, sports, etc.) and will not know your schedule until a later date, please write a note on the form and submit it by the deadline anyway. We need to know how many referees will be returning.  Once your other schedule is confirmed, you can email Mrs. Drislane.

Referee payment information:

U8 :  $15.00/game

U10, and lines (U12/U14) : $20.00/game

U12/U14 Center: $30.00/game

Games will be played 5/4/13 – 6/15/13, no games 5/25/13 due to Memorial Day Weekend.

Fall Payment schedule (every 2 games):

5/4 and 5/11 games will be paid on 5/15

5/18 and 6/1 games will be paid on 6/5

6/8 and 6/15 games will be paid on 6/19
